<html><head></head><body><h1 class="v3-editor-heading-1">개요</h1><div class="v3-editor-content-paragraph-helpline">배아의 발달 과정에서 척추의 기형 혹은 척추 분절 결손과 연관하여 발생하며 살아서 태어나는 태아 1000명 당 0.01~1.0명의 빈도로 발생한다고 보고되어 있습니다.&nbsp;<br><br>선천성 척추측만증을 유발하는 척추기형 중 척추뼈 형성 부전 (defect of formation) 및 척추뼈 분절 부전 (defect of segmentation) 으로 인해 선천성 반척추가 발생합니다. 완전분절 반척추 (fully segmented hemivertebra, free hemivertebra)인 경우 반분적 반척추 (semisegmented hemivertebra)보다 측만의 진행이 빠릅니다.<br><br><img alt="정상척추와 비교 그림" src="https://helpline.kdca.go.kr/cdchelp/img/dis_dtl/2022149001.jpg" class="v3-editor-content-image-paragraph-helpline-zoomable"><br><br>척추 형성 부전 및 분절 부전은 태생기 5주에 발생하므로 선천성반척추는 출생 시부터 존재하는 것이지만 척추측만증 혹은 후만증은 성장 과정 중에 나타나는 경우가 많습니다. 임상적 변형은 유아기에 발현하기 시작하여 사춘기의 2차 발육 급진기(adolescent growth spurt, secondary growth spurt)에 증상이 급속히 진행됩니다.</div><h1 class="v3-editor-heading-1">원인</h1><div class="v3-editor-content-paragraph-helpline">정확한 원인은 아직 알려져 있지 않습니다. 척추 단독의 문제로 인한 단일 반척추 기형이 발생하기도 하지만, 심장, 신장 및 척추관내 신경 계통 등 다른 여러 가지 선천성 이상과 함께 증후군 형태로 발생할 수 있습니다. 여러 가지 환경적 기형 유발 요인과 다양한 유전적 요소가 정상 척추 발생 과정에 함께 기여하는 다인자 관련 문제일 것으로 생각되며, RIPPLY2, ROR2, TBX2, TBX6 의 돌연변이가 연관되어 있는 것으로 알려져 있습니다.&nbsp;</div><h1 class="v3-editor-heading-1">증상</h1><div class="v3-editor-content-paragraph-helpline">발생하는 척추 변형의 형태와 정도는 환자별로 매우 다양하며, 선천성 척추 기형 환자에서 변형의 진행은 성장 속도와 관련이 많아서 출생 후 5세까지 그리고 10~14세 시기에 두드러지는 것으로 알려져 있습니다.&nbsp;<br><br>척추 변형이 심하지 않은 경우 평생 동안 경미한 어깨 높이의 변화, 골반 균형의 변화가 동반되는 환자들도 상당 수 있는 반면, 조기에 변형이 발생하여 고도의 강직성 변형이 발생하여 외관상 미용적 문제와 척추 균형 기능, 심폐 기능 저하 및 척추 압박 등의 문제를 일으키는 경우도 일부 환자에서 발생합니다. 약 60% 정도의 환자에서 신경계 기형, 심장계 기형, 비뇨기계 등의 동반기형이 있는 것으로 알려져 있습니다.&nbsp;<br><br><strong>◈ 중증소견</strong><br>조기에 진행하는 척추측만증인 경우 고도의 강직성 변형이 발생하여 외관상 미용적 문제와 척추 균형 기능, 심폐 기능 저하 및 척추 압박 등의 문제를 일으키는 경우 중증으로 분류될 수 있습니다. 또한 한 환자에서 2개 이상의 반척추가 같은 쪽에 나타날 때 그 예후가 매우 불량하며, 특히 정상 척추를 사이에 두고 발생하는 경우 2개의 진행성 만곡을 유발할 수도 있습니다.</div><h1 class="v3-editor-heading-1">진단</h1><div class="v3-editor-content-paragraph-helpline">단순 방사선 영상에서 완전분절 반척추, 반분절 반척추, 비분절 반척추, 완전분절 감돈 반척추를 진단할 수 있으며 추적 관찰 시에도 유용합니다. 반척추의 후방구조가 예후 및 진행에 영향을 미치는데 후방구조를 분류하기 위해 3D CT (three dimensional computed tomography) 촬영을 하는 경우도 있습니다.&nbsp;</div><h1 class="v3-editor-heading-1">치료</h1><div class="v3-editor-content-paragraph-helpline">지속적인 외래 추시 및 방사선 검사로 질병의 진행여부를 고려하여 치료 방침을 정해야 합니다.&nbsp;<br><br>보존적 치료로 보조기를 사용할 수 있으나 이는 만곡을 정지시키거나 교정할 수는 없지만, 진행을 늦추어 유연성을 유지시킴으로써 척추가 수술하기에 적당하게 성장할 때까지 수술을 지연시키는 효과는 있습니다. 수술의 목적은 불균형적인 척추의 성장을 정지시키는 것이며 척추융합수술을 시행한다.&nbsp;</div><h1 class="v3-editor-heading-1">참고문헌</h1><div class="v3-editor-content-reference-helpline">1. 석세일. 척추외과학 제3판. 2011;466-500<br>2.
